The primary purpose of the Clinical Program Coordinator position is to support and oversee the design, implementation, and ongoing management of clinical patient care programs. The associate will be responsible for overseeing the Medication Therapy Management program and helping support pharmacy associates in optimizing positive patient outcomes. Assists the Specialty Pharmacy and Call Center in the creation and on-going support of clinical patient programs for specialty medications. Responsible for identifying drug therapy opportunities across the department and creating evidence-based enterprise level programs. The position will support store based clinical questions by researching or using current clinical knowledge and aid in the establishment of protocols and policies related to the clinical patient programs.
The associate is responsible for the functions below, in addition to other duties as assigned:
Pharmacy degree from an accredited college of pharmacy, plus at least two (2) years’ experience in a community or centralized pharmacy practice environment. At least two (2) years’ experience completing MTM related activities and/or other clinical patient programs. Active license as a practicing pharmacist in PA and immunization certification through an accredited organization. Additional state licenses are preferred and will be required as needed.
